This policy covers the website at surrogacyinstituteofhawaii.com. It describes what we collect through this site, why we hold it, and who it reaches. It does not cover the information you give us later in a journey, which is governed by the agreements you sign with us.
What this site collects
The intended-parent consultation form
When you use the form on the homepage we collect your first and last name, email address, phone number, time zone, where you are in the embryo process, the name of your fertility clinic if you have one, your answer on whether you would like a referral to the Fertility Institute of Hawaii, and anything you write in the free-text box.
We use it for one purpose: to contact you about your enquiry. It is sent to us by email and is not added to any advertising list.
Please do not send medical records or test results through this form. It is a general enquiry form, not a secure clinical channel. See our HIPAA Notice.
The surrogate application
The “Apply” links on this site take you to a separate application system hosted by a third-party provider at surrogacyinstituteofhawaii.o-jms.com. Information you enter there is collected under that provider’s terms as well as ours. Nothing you type into that application passes through this website.
Server logs
Our hosting provider records standard request data, including your IP address, the page requested and your browser’s user agent. We use it to keep the site available and to limit automated abuse of the enquiry form. We do not use it to build a profile of you.
Cookies and tracking
This site sets no advertising or analytics cookies and runs no third-party tracking scripts. Fonts are served from our own domain rather than a font network, so loading a page does not report your visit to anyone else.
Who we share it with
- Our email provider, which delivers the enquiry to our inbox.
- Our hosting provider, which runs the site.
- The Fertility Institute of Hawaii, only where you have asked us for a referral.
We do not sell your information, and we do not share it for anyone else’s marketing.
How long we keep it
Enquiries are kept in our email system for as long as we are in contact with you and for a reasonable period afterwards in case you return. Ask us and we will delete yours.
Your choices
Write to surrogacyinstitute@gmail.com to ask what we hold about you, to correct it, or to have it deleted. Residents of some states have specific statutory rights; tell us which state you are in and we will apply them.
Children
This site is not directed at anyone under 18 and we do not knowingly collect information from children.
Changes
If this policy changes we will update the date at the top of this page.
